Clinical management of the painful diabetic neuropathies.

چکیده
340 Journal of the Royal College of Physicians of London Vol. 34 No. 4 July/August 2000 The neuropathies of diabetes are common, affecting up to 50% of older patients with type 2 diabetes. They have been the subject of several recent reviews1–3. Only a minority of patients experience neuropathic symptoms, so the absence of symptoms must never be equated with the absence of neuropathy1–3. A careful clinical examination of the lower limbs and feet is essential to diagnose neuropathy in any diabetic patient.

Mechanisms and Management of Diabetic Painful Distal Symmetrical Polyneuropathy
Although a number of the diabetic neuropathies may result in painful symptomatology, this review focuses on the most common: chronic sensorimotor distal symmetrical polyneuropathy (DSPN). It is estimated that 15-20% of diabetic patients may have painful DSPN, but not all of these will require therapy. In practice, the diagnosis of DSPN is a clinical one, whereas for longitudinal studies and cli...

PURPOSE OF REVIEW To summarize the current understanding of clinical assessment, pathophysiology, and treatment of pain in neuropathies, focusing on selected entities in which the understanding of the mechanisms underlying pain has advanced recently. RECENT FINDINGS Ongoing studies are classifying the symptoms and signs of painful neuropathies, assuming that this approach may indicate particu...
متن کاملTapentadol extended release in the management of peripheral diabetic neuropathic pain
Tapentadol, a μ-opioid agonist and norepinephrine reuptake inhibitor, has been found to be an effective medication for a wide variety of chronic pain conditions, including back pain, cancer-related pain, and arthritic pain. It has also been found to have fewer gastrointestinal side effects than more traditional opioid-based therapies. More recently, tapentadol extended release has been demonstr...
متن کاملSympathetic Blocks Provided Sustained Pain Relief in a Patient with Refractory Painful Diabetic Neuropathy
The sympathetic nervous system has been implicated in pain associated with painful diabetic neuropathy. However, therapeutic intervention targeted at the sympathetic nervous system has not been established. We thus tested the hypothesis that sympathetic nerve blocks significantly reduce pain in a patient with painful diabetic neuropathy who has failed multiple pharmacological treatments. The di...
متن کاملUse of anticonvulsants drugs for neuropathic painful conditions.
Neuropathic pain, a form of chronic pain initiated and sustained by an insult to the peripheral or central nervous system, is a challenge to clinicians as it does not respond well to traditional pain therapies. However exact pathophysiology is not known but considering similarities between epilepsy models and in neuropathic pain models justify the rationale for use of anticonvulsant drugs in th...
